The documents are listed in 4 categories:  New Players, Varsity Only, JVD/Varsity, JVD Only.

ANY NEW PLAYER TO THE DUCHESNE HOCKEY PROGRAM (grades 7th - 12th) 
 

  1. Copy of Birth Certificate 

  2. Plus the documents listed in the division in which you fall below.

VARSITY PLAYERS ONLY

  1. 2020-2021 USA Hockey Registration Certificate available at www.usahockeyregistration.com

  2. Copy of Current Health Insurance Card (front and back)

  3. 4th quarter report card from last school year (only players in grades 10th - 12th)

  4. MidStates Medical Consent Form (click here)

  5. DHS Player Registration Form (click here)

  6. DHS Player / Parent Contract (click here)

  7. DHS Medical Release Form (click here)

  8. DHS Financial Policy & Club Fees (click here)

  9. Midstates Varsity Consent Form
